Huge discrepancies also exist within countries, including Scotland where a boy born in the deprived Glasgow suburb of Calton can expect to live 28 years less than one born in affluent Lenzie, just 13 km (8 miles) across town, it said.

That is huge. Why?

I wouldn't think religious violence in Scotland. But you inspired me to Google and hey, Wikipedia (http://en.wikipedia.org/wiki/Calton,_Glasgow). Poverty violence somewhat, also heavy drug and alcohol use, and poor diet. And even mention of sectarian tensions.

Glasgow is actually fairly notorious for religious (that is sectarian) violence. See http://en.wikipedia.org/wiki/Sectarianism_in_Glasgow and http://www.timesonline.co.uk/tol/news/uk/article568214.ece . My Dad grew up in Paisley and he has some pretty hair-raising stories about Glasgow. One of his friends who also emigrated to Canada has a scar on his face from a Rangers fan who randomly slashed him with a razor blade when he was walking down the street. He was lucky to get away.
